what forms are needed to establish paternity when mother of child is unwilling to help?

   You need to file for parentage determination. You can call the clerk of court and ask if they have a form available to fill out and file.

   Some states will serve the papers for you and some make you serve the papers. I would suggest you have the sheriff serve the papers.

   After the DNA test you will need to ask for a Declaration of Parentage. You can use this to get on the birth certificate and social security.

   You then need to ask for visitation and/or custody.

   You can take her to court and the judge will force her to take the child in for a DNA test. It is painless. All they do is swab the inside of the baby’s and the possible dad’s mouth.

   There is not a lot of information here to go on, however really it is a DNA test which will cost about £500 and will involve a blood sample from baby and prospective father. However, the mother will have to consent to this, unless you go to court to try and force the issue. A good family lawyer should be able to give you the exact low down on how to go about this. Of course there is always Jeremy Kyle!!!

   I don’t know if this helps, but Rite Aid has an over the counter paternity test kit.
